This 2-page sensory checklist can be used to perform observations in the home or classroom.  The information is beneficial for an understanding of a child’s sensory processing skills in their natural environment. This is not a diagnostic tool but rather a tool to gather information. All individuals have a range of sensory preferences. It becomes a difficulty when the sensory input interferes with occupational performance.  Items are marked for low and high thresholds of sensory input.  There are areas for your notes as well as overall impressions.
